For clients who would like to use their private insurance to cover some of the cost of treatment, we can attempt to verify coverage and benefits. Please fill out the form provided and we will contact your insurance company. Verification is not a guarantee of coverage or payment.
Currently, Warriors Heart is in contract with many insurance companies. This means we will bear the risk if your insurance company doesn’t pay, we will not back bill you for costs not covered by the insurance company. You will only be responsible for the “client responsibility” and any additional services you may opt into.
Warriors Heart will do a basic verification of insurance for both in network and out of network companies. This does not entail authorization or confirmation. The insurance company provides a basic picture of what they may cover. We will provide you with what the estimated insurance contribution will be and determine what your cost or “client responsibility” will be to enter into treatment.
