    In consideration of the positive exposure and publicity associated with their effort, I hereby forever willingly release and authorize Warriors Heart, its successors or assigns, including but not limited to use my name, my image, my story, and any materials I may supply, in any media and for any purpose whatsoever, throughout the world, in perpetuity. I also, by this affidavit, do hereby swear that the claims or representations made by me are true to the best of my knowledge and belief as of the time of this filming, photographing, or other related media, and that I have any, and all, records necessary to prove such. I will advise you if my opinions or experience change.I agree that all reproduction and all copyrights associated with the above described information and media are and shall remain the property of Warriors Heart, its successors and/or assigns. I agree not to request or accept any payment of any other consideration in exchange for signing this agreement and for the use of any of the above photography, video or media materials.
